  • 1st , 2nd & 3rd Year MBBS Annual Professional Results.
  • PM&DC Registration Number (to be mentioned in the application)
  • Matric DMC / Certificate or O Level Equivalence Certificate
  • F.SC DMC / Certificate or A Level Equivalence Certificate
  • Character Certificate from the college. It must be mentioned that in which quota (open merit, foreign, GHQ W&R or any other quota) the student was admitted in 1st Year MBBS and student is not detained.
  • Attendance Record (after final selection) & result of any exam held
  • CNIC student & father
  • Domicile
  • Minimum 2 contact numbers (student and father)
  • Present Mailing Address (to be mentioned in the application)
  • In complete application shall not be considered / entertained

The application of a student who is not registered with PM&DC will not be considered .
